Ambient scribe for therapy progress notes

An AI scribe listens to a session, with client consent, and drafts a progress note the therapist reviews and signs.

What it does

Captures in-person or telehealth sessions and drafts a structured note in formats like SOAP or DAP.

Where the privacy line sits

Session audio and transcripts are highly sensitive PHI. Get written client consent first, read the vendor's BAA, check audio retention and model training terms, and decide with counsel whether anything created counts as psychotherapy notes.

  • PHI: BAA required. Touches protected health information. Needs a Business Associate Agreement if HIPAA applies.
  • State AI law. Some states restrict AI in therapy (for example, Illinois). Check every state you serve.
  • 42 CFR Part 2. May touch substance use disorder records covered by Part 2. Get counsel's review of consent and redisclosure.

What stays human

Consent conversations, reviewing every line before signing, and any clinical judgment.
